Mosaic is a Perth based service provider for people with an intellectual/physical disability. Contemporary, flexible, innovative and NDIS based services.

Mosaic Community Care it is our mission is to enrich the quality of life of people with disabilities. Established in 1992 by a group of community members interested in developing options for their family members which they believed better suited the individual. Mosaic provides shared living spaces in purpose-built accommodation with dedicated, experienced teams providing 24/7 support.
